When you call to schedule an appointment we will send you confirmation of the date and time of your appointment. We will include a patient registration form, medical history form, and HIPPA privacy sheet. We request that you complete all forms prior to your visit. If desired, the forms can be downloaded from this web site and brought to your appointment. This optimizes your time while in our office and helps us remain on schedule. If you have questions, please bring them to our attention so that we may assist you.
Your consultation will consist of meeting our staff, becoming familiar with our office and having a consultation with Dr. Borkowski. He will discuss with you your desires and concerns in a private, comfortable setting. An examination usually precedes his formulating a professional opinion as to how to best achieve your goals. We want you to be well-informed about the actual process of preparing for surgery.
Preparing For Your Surgery
After scheduling your surgery, we will send you pre-surgical instructions regarding your surgery. This will include instructions for pre surgical testing, the cancelation policy, surgical fees, smoking policy, and revision policy. Approximately two weeks prior to your surgery you will have a pre-operative visit with Dr. Borkowski to review your surgery and answer any questions you may have.
